Intake forms

Before your first appointment, you’ll be asked to complete the following forms:

  1. Health History Form – to record your medical conditions, medications, past injuries, and treatment goals.
  2. Informed Consent Form – confirms your understanding of Massage Therapy, including its nature, risks, benefits, and your right to withdraw consent at any time.
  3. Payment & Cancellation Policy – outlines accepted payment methods (credit card, e-transfer, or cash), cancellation expectations, and late/missed appointment fees. A valid credit card is required to reserve your appointment.
  4. Treatment Location Confirmation (for mobile massage) – ensures the correct address is on file for your session.

 

Arrival & Set-Up

If you’ve booked a mobile massage, your RMT will arrive about 15 minutes early to set up and ensure the treatment space is ready. Please prepare a quiet area with enough room for the table and easy access around it.

Initial Assessment

Your RMT will review your health history, current concerns, and treatment goals. If needed, simple assessments such as range of motion or orthopedic testing may be performed to better understand your condition.

Treatment Plan

Together, you and your RMT will discuss your treatment plan, including areas of focus, expected pressure, draping, and your treatment goals. You’ll have an opportunity to ask questions and provide informed consent before treatment begins.

During your Massage

You’ll be positioned comfortably on the massage table. Proper draping will always be used to respect your privacy. Your RMT will check in throughout the treatment to ensure the techniques and pressure remain comfortable for you.

After the Massage

Your RMT may provide feedback on the treatment and suggest self-care such as stretching or hydrotherapy to support your progress. If appropriate, a follow-up treatment plan may be recommended.

Payment & Reciepts

A valid credit card is required to reserve all appointments and will only be charged if our cancellation policy applies. Payments can be made by cash, e-transfer, or the card on file. You’ll also receive an official RMT receipt which can be submitted to your insurance provider for reimbursement.
